my mother is 74 years old and was just diagnosed with breast cancer in both
breast. she is in the hospital and is also having congestive heart failure
and is a diabetic. she is refusing treatment for both and refuses to also a
biopsy be done. please advice on what to do.

I think your mother has made her decision, but you could try to ask her to
have a biopsy for two reasons.
A. Breast cancer in older woman tend to respond to hormonal therapy and a
biopsy would give this information.
B. Have the biopsy to confirm the diagnosis so you can see proper screening
as a daughter.
Biopsy doesn't require anesthesia, and shouldn't interfere with her other
diagnosis.

Is your mother of sound mind? If so, that limits your options to
persuasion, of course.
Alex's advice is good. An add-on option might be to find someone in her
circle of acquaintence who's had a biopsy to tell her how minimally
difficult it is.
Do you feel she's making her decision because she feels that her combined
conditions have so degraded her quality of life that she doesn't wish to
continue? Is there an element of depression involved? If so, perhaps
someone (doctor) could evaluate her for depression and treat that (if she'll
permit it).
My heart goes out to you -- this is a very difficult and complicated
situation to find yourself in!
