Default Post IMRT PSA

2 months after completion of IMRT, psa came back 0.01. No lingering side
effects from treatment.

Peter, what sorry for being ignornt here, but IMRT after RP< when your
PSA (based on the sig), was staying/already low?

Anothr dummy question related to your Tx: LRP removed the prostate, so
after undetectable period, PSA came back in 2006 from where? There is
no prostate, so does it mean the cells have metastasized, therefore,
wouldn't chemo be the next step? Why IMRT since there is no more
"target" to radiate?

The cancer can escape the prostate and still reside in the prostate bed
(the area where the prostate used to be). Radiation can do a good job
of killing the cells if they haven't left the bed. The radiation can
hit the lymph nodes, seminal vesticles (if not completely removed) and
the fatty area around the bladder and rectum.

They can create beams to hit these areas and not dose the bladder and
rectum to any great degree.

I think hormones and chemo are mainly used when the cancer cells have
migrated to distant parts of the body.

no problem - good questions

As John already replied, cancer cells may have escaped the prostate and
found a home in the prostate bed - or elsewhere. When psa rose from
undetectable to measurable, my last shot at a "cure" was 1) that that is
where the cancer was, and 2) the radiation would finish the job. IMRT
focused on a small area - maybe the size of a softball or smaller. So far
so good.
