Default Two new PCa treatment in clinical study

I have not decided which treatment option to take (DXed a month ago),
but in my research, there appear to be two new methods being worked
that may soon be available:

1. Tookad, which is in clinical study for RE-CURRING cancer in MUHC,
Canada, and
2. HIFU, already approved in other countries, and now in clinical stage
in the US (stage II/III), for PRIMARY, localized case.

Is there anyone here planning on doing the HIFU?

studied HIFU, but was unable to go through with it because of calcification.
I don't recall anyone else in the NG having opted for HIFU.

I had an appointment in Houston and saw Dr. Brian Miles for the HIFU
clinical trials. He recommended that a man of my age (54) would not be
a good candidate for the trials because of the government guidelines.
In other words, they would have to over treat the area.

My suggestion is that if you are interested in HIFU, contact the
nearest clinical trial in your area.

There is now some 5-year data available on HIFU. Just go over to
PubMed and search something like "HIFU, prostate cancer" and you'll get
a lot of hits. Look for the 5-year studies and you'll find information
on disease-free rates and side effects. Uchida and Gelat are two of
the longer- term practioners.

There was a fellow in this newsgroup who had HIFU. Search
the archives for "hifumike" to find him.

I don't know if HIFU is more or less effective than radiation
or surgery, but I would like to caution you that newer does not
necessarily mean better. Many new treatments come and,
after experience has been gained with them, go again.
Radiation and surgery have been around a long time because
they have been known to be effective for a long time. They
have also been continuously developed over time with new
techniques like nerve sparing surgery, laporoscopic surgery,
intensity modulated radiation therapy, and so on.

If you are attracted to HIFU for some reason that's fine. But
be sure that you don't assume that just because it's newer it
must be more effective, or that the side effects are less.
Evaluate the therapy and the practitioners just as carefully
as you would surgery or radiation, and be sure that you also
consult with a surgeon and a radiation oncologist before
making a final decision.

That's 2 cents from a guy with no credentials but who offers
heavy discounts on the price of advice

I've chatted with a fellow from Great Briton that is considering HIFU. He
states that his URO has done many surgeries and is pro-HIFU. He says there
are two systems, one developed in France-the original Ablatherm and the one
developed in the US-the Sonablatte. Both have there plus and minuses. HIFU
has been available for some time outside the US and apparently has been
quite successful so far.

Ron S...What do you mean by "quite successful" and how does that
compare to RP and RT?...ron?

Hi Ron,

I was afraid you would ask. Typing is somewhat of a chore for me because
of my MMN (motor neuropathy of the left hand. He has been trying to decide
between open surgery or LRP but his URO is HIFU certified and he may go
that route. His father was diagnosed in his fifties and was treated with
stilbeostrol and lived another 30 years. He states that they have done
about 300 with only one that had a problem. That ones bladder sphincter
and prostate were one and the ultrasound destroyed both so he is waiting
for a sphincter replacement.
They also catherise the bladder directly above the pubic bone and not
through the uretha thereby avoiding any incontinence problems. They can
usually save one nerve bundle and sometimes two. They claim erections are
better than surgery or radiation. I don't have any info as to the validity
of this claim or if it is a marketing claim. I guess only time will tell as
it is fairly new treatment. Total time from prep to cleanup is from 2.5 to
3 hours.

I met with the Doctor from France who helped to develop it 5 years ago (is
he a personal friend of a good friend of mine) and decided not to go to
France and have it done; settled for seeds and IMRT.
