| | | Jaw Bone Metastasis???? Hi,
For a while now I have had tooth ache, I made it to the dentist who
performed a root canal and took an x ray last week, the bone showed up
as a dark shadow around the root. I have been returning to the
dentist every couple of days and he has been putting antibiotics into
the channel. Today I went to see my Dr. and asked him if the problem
on my jaw bone could be a metastasis, his reply was there is a slight
possibility.
After seeing the doc I went to the dentist for another appointment, he
was concerned that the pain is still present, but pleased that it is
somewhat reduced, then he told me he is concerned that it may be a
metastasis, and that maybe why the pain has not gone yet.
Aside from the cleaning of the channel every 2 days I have been
prescribed Rodogyl tablets to clear any bacterial infection.
If the infections cannot be cleared the tooth will have to be
extracted.
I am completely freaked out by the possibility of a growth on my jaw
bone and my face.
Does anyone know how high the possibility is of this occurring I have
adeno histology, and cancer of unknown primary, with known mets on
lung, kidney, and possibly spine.
If I do grow a tumour on my face is there any way to treat it so I am
not disfigured, how rapidly would it grow?
I await words of wisdom I am really disturbed by this, physically, I
already look about 15 years older than I did a year ago and a visible
facial tumour would finish me off.

Hello isi,
Cancer can grow anywhere.
I've never had a root canal done, but my sister did and it was quite
painful for quite a while, if I recall correctly, then had to be redone,
although they managed to save it.
Em's mother had cancer in her jaw and teeth surgically removed, if I
recall correctly, then chemo and radiation therapy. She was in her 80's
and I've since seen a photo of her and she looks pretty good for an old
codgerette.
I don't know if a tooth should be pulled if there's a chance of mets, in
the jaw, or what scan should be done, so hold that thought until Steph
rings in with his comment, please.
Meantime, I hope the antibiotic improves things for you.

I have had several root canals and they are very painful. I have had
one dental implant which was a piece of cake compared to the root
canal. I can tell you an infected tooth is very painful... if you have
it pulled and the pain goes away that would answer your question about
mets.

Dear Isi,
Just wanted to reach out to you since I am a person who shares with you
being "freaked" out by the changes in my face since dental extractions (have
five front teeth left !) and extreme weight loss, and chemo and radiation. I
strongly resisted, at first, my primary Oncologist's advice to have all but
the front five teeth extracted, and the concurring opinion of a professor of
oral surgery at the local medical school (both U..S. trained and board
certified). But they won me over with the idea that I might have to go to
Bangkok and spend some time in a hyperbaric oxygen chamber at great expense
if anything went wrong during radiation with my teeth.
Two responses : I hope you will be able to get a more definitive diagnosis
of the actual condition of the dark area under the jaw soon. And also I
think, without having the scientific background to really know, that given
the state-of-the-art of modern plastic surgery, that facial conditions of
almost any type can be corrected with cosmetic surgery. My belief is that
the timing of such surgery would be conditional on your general state of
health, whether or not you are currently undregoing radiation, chemo, etc.
In any case, I hope you find the responses of concerned people in this group
helpful in getting the "big picture" back in focus and helping you worry
less as you await more accurate information, as I do.

Isi,
I really hope it's just the tooth problem. Some take awhile to resolve.
I don't think I'd have the tooth pulled unless it goes bad & can't be
saved. Yes, it might help answer the question, but it sounds a little
extreme to me.
Your immune system may be compromised too, making your recovery longer.
Please keep us posted. I hope it resolves soon.

Just so you know, Uncle Sally.
The big picture is that isi has lung cancer with metastasis and is not seeking
treatment such as chemotherapy. Her cancer is advanced.

Thank you so much for your replies, I am heartend to hear that this
sort of tooth problem can carry on for a while - I really hope it's
just a case of slow recovery.
My system is not working very well when it comes to getting over thing
like colds and bruises so I guess the same may apply to an infected
tooth. It still hurts and now the pain is going along the jaw into my
ear. This pain must be quite strong as it has a high intensity and I
am taking 360mg of DHC a day and also a couple of paracetamol to
reduce inflammation, so I really don't think it's an average
toothache. Back to the dentist tomorrow, I will ask if it's possible
that there is still some nerve left in the tooth. I would like to
save the tooth if possible it's one of my main biting tools.
If it does turn out to be a metastasis, how would they confirm that,
biopsy? As you know I am in Bulgaria, the medics here have very good
brains but the technology is a little (a lot) behind the UK. I came
here from London and was diagnosed shortly after arriving. Would I
need to have a lots of radiotherapy sessions? Or is that impossible to
say without examining it. I have to travel to Sofia for this and it's
an eight hour drive round trip.
My primary cancer has not been found, I have mets in lung and kidney

keep fit, do try to keep going even if you have to do less than you
would wish. This disease eats away muscle when your not looking, I
think that was my mistake. I used to dance so had good muscle tone
but not anymore, even my yoga is a bit of a struggle these days. I
let the yoga practice slide during a health crisis, (breathing
problems) which lasted for about 10 weeks during the winter. Normally
this would not be a problem and I would have been able to rebound
quickly, but not this time. Its like a tin man from the wizard of oz,
mime. You gotta laugh lol.

Isi, try wrapping some ice in a plastic bag with a facecloth cover. Rest it
where the swelling is for no more than 15-20 minutes. Ice reduces swelling
very nicely. Do this every couple hours and see if it dosen't relieve you of
some pain and swelling.
I used this a lot when I got sinus infections.

Isi, I sympathize with your situation if you are an "expat." In my
case I speak Thai fluently, and have greater trust in the health
care system here (where my US $ can buy the very best), than
I do in the US system. While not knowing what your constraints
are, I do hope you will be able to find a way to stay close to the
treatment center to reduce the stress of that travel !

fyi : the bone scan I had here in northern Thailand cost me about
US $150 and was done on a last year's model scanner. My doctor
told me it was "optional" in my case, but I felt it was a cheap way

In the US we have dentist who are called oral surgeons, they
specialize in more complex dental issues. ( for example if your jaw
had to be reconstructed they are the ones who put you back together
again). I would think a bone biopsy would be done right where the
infected tooth is.
The most acute pain I ever had is dental pain....worse then abdominal
surgery or childbirth. Is there a dental school near you they tend
to have the most updated equipment and dentists. My experience is
that once the tooth is pulled the pain will go away..... Alex |

Hi J
Thanks so much for looking into Bulgarian radiotherapy for me,
I am not too sure how sophisticated the equipment is here, but
I do know there is not a lot of it as one has to travel to big cities
for almost everything.
I am now contemplating leaving this small town I have landed in
and heading towards the capital Sofia and finding a home in the
country there - I am not sure yet, it's a lot of effort and I am
tired,
also would still have the language problem.
Part of me thinks of returning to the UK just because of the
language,
I don't really like the place. I was diagnosed in the UK with CFS
ten years ago and informed I was depressed, I knew there was
something else wrong with me. I kept returning to the Doctors
and asking for tests and help to become well again.
The attitude of the doctors there was patronising, they thought
they knew best and said it's depression, writing out a script for
more pills, this went on for years.
I quit taking the anti-depressants without telling the doctors,
but was still very tired. I thought it may perhaps be due to the
stress of living the UK, city life and all that.
So I sold up and moved here I did start to feel more
energised and generally stronger.
However my knees were painful and walking became difficult.
So I went to see a doctor here in Bulgaria a few weeks after
I arrived, he immediately saw that I was very ill, and ran loads
of tests, and the cancer was discovered also small heart problems.
The doctor here is astonished that cancer was not picked up by the
medics in the UK as I saw my GP there once a month.
All the doctors and professors I have seen here say I have had this
cancer growing inside me for many years and that its impossible to
miss if one has medical training.
My feeling is that if I returned to the UK and did not tell the
doctors that I have cancer they would never even think of it.
I have all the x-rays, scans and biopsy reports that were performed
here. It was so weird when I was diagnosed I really did not believe
it so arranged to go to another hospital in a different town with my
symptoms and nothing else, there I pretended I did not know what was
wrong with me and sure enough I was diagnosed again after another CT
scan.
So that's why I am highly reluctant to return to the UK. A very close
friend is in the States and we tried for me to go there, she has
offerd take care of me during the last days, but the maximum visa I
could get is one year, then I would have to leave the country. If I
knew for sure that I was going to be dead within the year I would go
to the US, but I don't know that. If I was very ill but alive I may
not be in any condition to exit the US get a new visa and return
there. So it's all very difficult.
Wow I am really pouring out my problems today.
I did not see the dentist yesterday, as I awoke with severe abdominal
cramps, my liver felt like a rock and my urine was really dark, so I
stayed in bed with a heating pad on my tummy. I think perhaps it's
the Rodogyl as I felt weird an hour or so after taking the first
dose. So I have stopped taking them, have now missed three doses and
do feel a lot better. A coincidence perhaps, when I have
'normalised' I may try them again.
The ice Betsy suggested has helped take the fire out of my jaw and
ear. It still hurts but not as much thankfully, have now arranged to
see the dentist again tomorrow.
This has been rather long - I am sorry about that, I wanted to fill
you in a bit more about my situation and history, also I sometimes
feel isolated here and need to express what's going on.

Thanks for your input, it's always good to
know how to go about these things, and be
prepared I am praying it's just an
infection on the bone, and that is't
clearing up. It is a little better day
by day so fingers and toes crossed
it's not another metastasis.

Rodogyl, I think, is a combination of Spiramycin and Metronidazole (Flagyl)
I think I saw an "abdominal pain" side effect on the combination.
Spiramycin has a major warning for a different medical condition.
Dark urine can occur with Metronidazole
Your liver took a "hit" from the antibiotics (?) or me wondering if your weight loss is
because the cancer's in your liver .(?) If so, you might be better off going to US
friend?
Sounds like your cancer's quite advanced, weight loss and loss of stregnth and so
fatigued.
If not your liver involved (and it doesn't always show up on scans, at first), well,
look at the map, on the Adobe file and see where you prefer, or if Sofia, you know for
sure has the care you would need, then I support you're going there.
If liver's involved, time could be very short. Unless you have co-existing liver
problems and/or the other medicines you're taking have a liver warning. If not, if the
urine continues dark for more than a week (I would say) there's something very wrong
with your liver. Are the whites of your eyes (and skin) yellow?

Watch for Steph's reply. Could your neighbor call the cancer centre in Sofia to ask
about RT treatments? And who would be with you, in Sofia, in the end days, weeks,
months.? Is there hospice care there? But there's the language problem.
It's so hard for us to know what's happening in you, not being there.
Maybe your US friend would be better if she can get you hospice care or medications
where she is.
(sorry, I can't make up mind on this either, but communication is important) but I don't
know how much time you have. If your urine's okay and no Yellowing" maybe you're okay
for a while yet, but I don't know if it's safe for you to fly.
I' m sorry UK doctors let you down, isi.
