| | | A1c Question & Rant You all are pretty good experts and I wanted to ask a question. What is the
possibility that my A1c could go from 12.6 to 6.1 in 3 months if it was only
the medicine (Metformin 500mg and Januvia 100mg), and me not doing
"anything"? I am so hurt the doctor thinks I am not having success because
my WEIGHT is not down enough for her liking. It's all about weight to her,
and I absolutely agree this is a major problem, but I AM working on it very
hard. I am down 20 pounds since March (and that includes losing the 12
extra pounds that the last med caused me to gain last month when I was taken
off of it, in addition to the 20 I had already lost...so really, it's 32
pounds lost since March). Also, my labs showed my triglycerides down 200
points, cholesterol down 30 points, good cholesterol up 8 points, and BP was
down 30 points until all this sickness and pain set in, and now it is up
there again. I have changed everything about my life including going from a
couch potato to getting some form of exercise daily. I have been sick the
last couple of weeks and have not been well enough to do anything, but
yesterday, when she told me I was using the diagnosis of "diabetes" as an
excuse to not try, I felt like running out of there. You guys have heard me
share about checking out 13 books at the Library, going to support groups,
joining the American Diabetes Association, cleaning out my cabinets and
putting health food in there, taking diabetes education classes, meeting
with the dietician, my own online research a few hours a day, getting a
pedometer and walking a little everyday even though I have fatty tumors and
other feet problems which I am still waiting for a referral to the
podiatrist. I have bad fibromyalgia and arthritis in my hip, but I still do
it. Tears happen, but I am moving that body no matter what because I want
to get better. I am devastated by what I feel is a low blow by a doctor I
thought I could trust.

400 points without any extra work on my part (also the other lab numbers
coming down without other meds to help or me just sitting on a couch (which
is not the case)...just those two diabetic meds). At diagnosis, I had some
readings in the 500's, and in the last 30 days, my highest was a 160, and
that was only once. Just checked my meter and for last two months, my
average has been 118 this month and 114 last month.

I'm not the expert you probably expect. I'm a newbie at 1 year, myself
but.... Wow.... losing close to what, 10 pounds a month?... sounds like you
got the bull by the horns to me. Those bg numbers, as well as the others,
don't look too shabby either... considering where you started from. Seems to
me you're well on the road there. I think you deserve some credit.... and
maybe a different doctor.

Meds are designed to be used in conjunction with diet and exercise, but...
I do believe that taking meds will lower BG some for some people. If it
didn't, you wouldn't see so many diabetics who just eat whatever they want,
whenever they want.
As for the weight loss, I wouldn't worry about it. It may or may not
happen. I know Drs. are big into weight loss. I was praised for weight
loss that came about from dehydration and being so sick I was unable to eat.
I complained to the Dr. who merely said he was very proud of me. For what?

I think your doctor is nuts. Even just 20 pounds in 3 months is
plenty, not to mention the other weight. I am a firm believer in slow
weight loss because I think it is easier to keep off slow weight loss
than quick weight loss. I believe in setting up a lifestyle that will
be for the rest of your life, and quick weight loss is not a lifestyle
that anyone should do for the rest of their lives.
You're doing better than great. Sometimes doctors really get on my
nerves.

Dear Kris,
You are understandably upset by what your doctor has said to you.
What I tell my diabetic patients is that VAT loss is the key to better
glycemic control and that any major improvements in HgbA1c will be
credited to less VAT: http://HeartMDPhD.com/HolySpirit/Healing
As for an acceptable rate of VAT loss, it would be 3-5 lbs per month.
Your losing 20 lbs of VAT in 4 months would be an acceptable rate of 5
lbs per month if you were my patient.

Unless you weight 900 pounds...losing more then 8-10 pounds isnt a good
thing as you body will try to revolt and yo-yo you back up..losing the
weight slow and steady allows the body to accept the new you and more chance
of keeping it off.
As the others say, you might look for a new doctor as you can probably hit
you goal weight and that doesnt mean your dibaetes is cured..thin people can
be type two...granted losing body fat is a big help as fat helps insulin
resistance...but the fact he ignores your excellent progress and is obsesed
means he or she probably is a one trick pony that you may want to be rid of.
just my opinion.

Losing 20 pounds is good if your beginning weight was 180-200 pounds, but if
it was 250 or more, then it's not all that wonderful, is it? How much do you
weigh now, and how much does your doctor think you should weigh to be
healthy?
Your doctor does sound a bit harsh, but we pay doctors to help us improve or
health, not be our friends.
My doctor was on me about losing weight and I did. I lost 50 pounds in 4
days while laying on my back in the hospital with congestive heart failure.
Perhaps your doctor sees such an event in your future and is pushing you so
hard to try to avoid it.
If you truly think your doctor is treating you too harshly, then maybe you
can go to a different doctors. |

I agree, John... Kris is trying hard, and doing a lot of what seems to
be very right. You deserve great credit, Kris, for having made such
significant progress. By the way, if you think you may have Cushings
of some other endocrine related problem, you owe it to yourself to get
checked for that. It could make a huge difference in your overall
quality of life.
Maybe the doc is pushing too hard, or perhaps not very sensitive to
the human side of the needs of the patient.... Also, maybe you need a
specialist.

As a interesting sidenote most all the people on the biggest loser who went
on those extream wieghtloss shows are all yo yo'd back up..its better to
lose slow and steady and eat right and exercise then obses over the scale.
Did the doctor do a BMI test?
At least your advice at the end was good in that if she feels uncomfortable
she should see a new doctor.

Evidently you don't know what congestive heart failure is, among other
things.
Congestive heart failure is caused by a build up of liquids in the body due
to a number of causes, the most common being the consumption of too much
salt in the diet
I was given IV diuretics which cased the water to drain off and thus the
loss of the 50 pounds.
I suggest before you respond to any other post, you do a little research to
learn at least a little of what you are ignorant about. |

While A1C is obviously important to a lot of us here, maybe it gets
overemphasized in this group compared to all the other possible
measures of health (even diabetic health).
Some really good docs just like raising the bar all the time to keep
their patients moving in the right direction. Just like a good teacher
will give harder assignments and expect more from their better
students.
At the same time, some docs will just give the same "do better" speech
to everyone and not actually help them move in the right direction.
Sometimes it truly can be difficult to tell the difference. I'd assume
the more optimistic for now. And despite the raising of the bar,
you've actually done incredibly well so far - congratulations!

Sure, A1C can drop that much. Older wisdom was that it represented an
'average' value over the last three months or so. I recall reading fairly
recently that research and a better understanding of the mechanisms
involved indicate it may be better described as a 'weighted average' over
the last few weeks - heavily weighted by the last week or two. I saw a
similar drop after I was diagnosed - although that was with D&E and
metformin. |

you know it's a TRIAD for treatment of diabetes........ diet/exercise/meds
in that order

ps. do you have a swimming pool where you can sign up for aquasize? it will
give you body support easing the weight on the feet, while giving your
muscles resistance that you are exercising against....... excellent/easy on
the body/ exercise regime to get into

Hey Kris, I've been following your posts (albeit a little haphazardly)
and my impression is that you have been working very hard to modify
your lifestyle and doing very well. I believe most weight loss
advisors suggest losing about 2 pounds a week max. You've done that.
Imo, your weight loss, and lab results reflect your hard work. Sure,
the meds are going to help with all of these things, but they are only
piece of the picture. You could take these meds, eat cake and candy,
sit on the couch all day long, and you would not get the weight loss
and improved labs that you have now.
Your doctor was off-base. If you've had a good relationship with her
up to this point, you might consider giving her another chance. Who
knows, maybe she had a fight with her husband that day. The
unfortunate truth is that doctors are human too and can be idiots just
like anyone else. I used to work in a doctor's clinic and have seen
doctors that were truly good, caring people occasionally say the
stupidist things. It just sucks that your doctor had to pull that
crap when you really deserved and needed that pat on the back and word
of encouragement. If you can't get past it, however, or she makes a
repeat performance, then I wouldn't hesitate to look for another
doctor.
I'm sorry you're not feeling good now. I'm sorry I must have missed
the post about the nature of your current illness. I hope it resolves
soon.

Howdy Kris,
It's possible that your A1c could have gone from 12.6 to 6.1 doing
nothing at all but take meds. But since you did a lot more than just
take meds, you were doing more than many people that get diagnosed
with diabetes do. They pop the pills (if they take them) and hope
that taking them does everything, without them having to make any
changes or adjustment in their lives or diet. Guess what, you're not
one of them.
Now having said that, it's not very likely that making no other
changes other then taking a couple of pills would have dropped your
A1c that much. But then you will never know, since you did much more
and made changes and adjustments in your life and diet.

Make a list of the changes you have made and what you are doing, all
the changes and adjustments you have made in your life and your diet.
Ok time to stop being hurt and start looking her in the eye and tell
you what you think.
Then ask, "Since you feel I'm not doing enough, what should I be
doing, other than what I have been doing?" Ask her to please answer
directly and in detail as she did when she said to you... "You are
not having success because your WEIGHT is not down enough my liking"

that comes with it (or them if more than one), and the meds and side
effects (if any).
Always remember, other medical problems, being sick and pain are often
great Unmotivaters, but working through them as you say you have been,
proves that you are doing more than don't nothing. Could you do more?
Only time and you can tell us that.
So tell your doctors and anyone else that shares with you, that YOU
are not doing as well as THEY FEEL you should be doing, that until
they can do what you HAVE been doing, to keep that type of advice
buttoned up in their ass and to only allow it out when you ask for
their best ASSVICE. <wink>
