I don't know about one called Health, but I like GlucoseOne which is
free and does all of this. Plus it tracks carbs and other things as
well.

While GlucoseOne does blood pressure too, I like the chart on BpDiary
better. Well GlucoseOne doesn't have a chart for blood pressure. It just
lists data.
