I've been on Januvia (100 mg/day) since early January 07. Dropped my
meter reading average from 190 to 155, but only changed the A1c from
8.1 to 7.9 (measured at 3 months from start of Januvia). Must be the
overnight unmonitored highs. Very disappointed. Also on
metformin 2000
mg/day.
So now Amaryl (glimepiride, a 3rd generation sulfonylurea) has been
added (2 mg/day). That seems to be making a big impact, but again it's
only my meter numbers so I'll have to wait for another A1c in a couple
of months.
Of course the problem with Amaryl is the lows. Not having clinical
lows, but when it hits the 70's or 80's I notice it - jittery & visual
changes. Maybe it's because I've been so high for a while and my body
has adapted to those numbers.

Here's some history that I posted back in February:
Been dx'd T2 for 11 years. First on diet & exercise only, then
increasing doses of metformin. Maintained A1c from below 6 to maximum
of 7 all that time, but past year it was on northerly slope. Tested at
8.1 in Jan 07. Had tried
Actos briefly but not much effect and didn't
care for the side effect/risk profile.
I switched to an endo and was expecting to be put on
insulin. But
after explaining the classic 5-15 year T2 beta cell burnout, he
starting talking about the "paradigm shift" in T2 diabetes treatment.
I don't know if that is because of the DPP-4 inhibitors or some other
change in thinking. Suggested I try Januvia.
Only noticeable "side effect" for me is slowed GI tract, actually part
of the proclaimed benefit (lower PP peaks). Could tend toward
constipation if not for the opposite effect of the 2000 mg/day of the
metformin I am also taking. No evidence of the only "official" side
effect of sore throat/upper respiratory inflammation.
