Default Lab tests..

I got my lab tests from my last endo visit. The results were generally good:

A1c - 5.5, down from 5.9
Cholesterol - 150 up from 137
Triglycerides - 97 down from 123
LDL (Calc) - 76, up from 62
HDL - 55 up from 50

One thing that worries me is a note my endo wrote on the test sheet, she
circled my A1c (5.5) and wrote, "much lower than logbook would have
suggested. NV check Fructo (Fructosamine)." Is it common for some people to
get inaccurate results from the A1c test and then they do the Fructosamine
test?

I think she's just being diligent. Your A1c is just an average, not a
test of your actual bg levels, and she's looking for further evidence of
your overall control.

Meanwhile, those lipids are GREAT! In fact, your TGLs suggest your
control should be greatly improved.

Hi Anon,

Looks to me like you are doing very well.... You have numbers almost
anyone of us would be pleased with.

Of course I'm very happy with the numbers, except the higher Cholesterol and
LDL. I was just wondering why she is questioning the relevance of my A1c
enough to do the Fructosamine.

It could be because she wants to monitor recent changes in my control. I did
replace Novolog and Starlix with Byetta 2 weeks ago. I'll get a chance to
ask her about it in my next appointment in 2 weeks.

I would say you are probably right... From what I understand about the
fructosamine test it is more a measure of average control over the 2-3
weeks prior to the test sample being taken.

Three years ago I went on the Atkins diet, 20 carbs/day. I lost 45 pounds in
3 months and my Triglycerides was below 90. After I went on meds and started
eating more carbs, 90-120/day, they went up to the 150 range. It is only the
last 6 months they have started to go down. I started using Novolog before
high carb meals and was eating an average of 180carbs/day. It must have been
the insulin and tight BG control that lowered them again.

Looks good to me. Don't forget that 5 points of that 13
increase in total cholesterol came from the HDL - which is
good. Also your trigs/HDL ratio improved dramatically from
2.46 to 1.76 and both ratios are less than the goal of 3.0.

What "logbook" entries does your endo see? One hopes that he
is aware that at low A1c's your post-prandials are more
relevant than your FBG's.

Actually - "looks good" is an understatement. Those numbers
look terrific.

It happens. Certain kinds of anemia, especially, can throw it off.

It's also true that one's logbook will reflect those occasions when one
tested, which for some folks might contain mostly times when one's BG
was high and one wanted to know *how* high. Many people won't be
testing during the long times when one's tooling along in a fine old
normalish range. One's A1c is analagous to a reflection of *all* the
time, though, not just those higher times.

Has your clinician ordered the fructosamine test? It might not be a bad
idea.

I don't know how common it is, but the fructosamine test can be used
as a cross-check on the A1c. It correlates with BG, but over a shorter
period than the A1c.

My endocrinologist has ordered a fructosamine test on a couple of
occasions when he thought the A1c was too *high* based on what he saw
in my logbook. This turned out to be the case.
