What is quaternium-15 and where is it found?

Quaternium-15 is a formaldehyde-releasing preservative.
It is used in many cosmetics and pharmaceutical preparations.
It is also used as a preservative in many commercial and industrial
products.

This study presents a 10-year survey of 2285 patients tested to common
preservatives in northern Israel.

The demographic and clinical data were analysed using a revised MOAHLFA
index.

Patch testing was based on the International Contact Dermatitis
Research Group (ICDRG) guidelines.

411 (18%) patients were found to have positive reactions to 1 or more
preservatives.

The most frequent allergens tested positively were
thimerosal,
methylchloroisothiazolinone/methylisothiazolinone (MCI/MI), and
formaldehyde.

The reactions to MCI/MI remained stable around 1.7% throughout the
years 1995-1998,
with a significant increase to 3.7%-4% (P = 0.029) during 1999-2004.

The reactions to formaldehyde remained low around 0.3% until 1999;
however, a considerable increase throughout the years 2000-2004 was
noted, with rates of 1.5%-1.9% (P = 0.028).

The sensitivity rates for parabens,
quaternium-15,
imidazolidinylurea, and
diazolidinylurea
were low and unchanged, mostly under 0.5%.

As for methyldibromoglutaronitrile (MDBGN),
sensitivity rates around 2% were detected for the years 1996 and
throughout 2000-2004.

Overall, this extensive survey showed a concerning increase in the
sensitivity rates for MCI/MI, formaldehyde, and MDBGN.

BACKGROUND:
Allergic contact dermatitis is a significant cause of cutaneous disease
affecting many individuals in the home and at the workplace.

Patch testing is the most worthwhile diagnostic tool for the evaluation
of patients with suspected allergic contact dermatitis.

OBJECTIVE:
This study reports the results of patch testing from January 1, 2001,
to December 31, 2002, by the North American Contact Dermatitis Group
(NACDG).

METHODS:
Patients were tested with an extended screening series of 65 allergens.

A standardized patch-testing technique was used.

Data from these patients were recorded on a standardized computer entry
form and analyzed.

RESULTS:
Sixty-five allergens were tested on 4,913 patients.

The top 10 allergens remain the same in this study period as in the
1999-2000 study period:
nickel sulfate (16.7%),
neomycin (11.6%),
Myroxilon pereirae (balsam of Peru) (11.6%),
fragrance mix (10.4%),
thimerosal (10.2%),
sodium gold thiosulfate (10.2%),
quaternium-15 (9.3%),
formaldehyde (8.4%),
bacitracin (7.9%), and
cobalt chloride (7.4%).

Of the 4,913 patients tested,
69% had at least one positive allergic patch-test reaction.

Of all patients,
15.8% had occupation-related dermatitis;
15.4% were determined to have irritant contact dermatitis, and
11.1% of the 15.4% had a relevant reaction to an occupational irritant.

Of all patients tested,
16.7% had a relevant reaction to an allergen not in the NACDG standard
series, and
5.5% had a relevant reaction to an occupational allergen not in the
standard series.

CONCLUSION:
Our findings once again reinforce the need for a more comprehensive
group of diagnostic allergens than those found in the standard
screening kits.

The usefulness of patch testing is enhanced when a greater number of
allergens are tested, especially nonstandard allergens occupationally
encountered.

Dark wines and liquors, as well as aspartame, provide
similar levels of methanol, above 120 mg daily, for
long-term heavy users, 2 L daily, about 6 cans.

Within hours, methanol is inevitably largely turned into formaldehyde,
and thence largely into formic acid -- the major causes of the dreaded
symptoms of "next morning" hangover.

Fully 11% of aspartame is methanol -- 1,120 mg aspartame
in 2 L diet soda, almost six 12-oz cans, gives 123 mg
methanol (wood alcohol). If 30% of the methanol is turned
into formaldehyde, the amount of formaldehyde, 37 mg,
is 18.5 times the USA EPA limit for daily formaldehyde in
drinking water, 2.0 mg in 2 L average daily drinking water.

Any unsuspected source of methanol, which the body always quickly
and largely turns into formaldehyde and then formic acid, must be
monitored, especially for high responsibility occupations, often with
night shifts, such as pilots and nuclear reactor operators.
