| | | A question I am sorry to be a pain again :-(
I had a voice mail from my endo's office and she said that people with
recurrent UTI 's or Kidney infections should go to the primary care.
I am confused because she flat out told me that I am getting them on a
recurring basis because of my diabetes being uncontrolled and the primary
care is the one who said he would not treat my diabetes anymore so now which
way do I go? If it is diabetes related why would I be told to go to the
primary care? How do I know who to go see and for what now?

infections with your diabetes. I have had them since I was 22 y/o. I am
64, and still have them (have one right now, in fact  , even taking a
prophylactic antibiotic. I see a urologist for my urinary problems (you
should see your pcp first and go from there), and my endo for my diabetes.
I have six different doctors for currently active diseases right now. They
talk to each other, but each one controls each specialty. I am glad to hear
you endo referring you back to your pcp for your recurring utis.
If you want to talk utis privately, I'd be glad to.

Hi Cheri
I have Humana PPO through my husbands work and I have Medicare through SS so
I have good insurance for a change. I feel like no one wants to take care of
me. They say the infections are diabetes related then don't want to care for
me and send me back to the doctor who told me to go away to begin with. See
why I stay away? I never have anything good to tell you all. I can't keep
burdening people especially good people like you all , all the time. I just
don't know where to turn anymore. |

Hi Billie,
Thank you for being so kind. It was the endo that found the infection a
month ago as two months before I thought I had one as I had the symptoms and
my pcp told me it wasn't not one but then a month later I went to the endo
for regular visit and she told me I did have one and gave me two weeks of
antibiotics. Then went for two more weeks or a month and then said I had
another one and this time yesterday when the endo's nurse called told me
they are diabetes related because my bg's have been very high and
uncontrolled for over a year now. Thanks for your information and I feel bad
for you to have to see so many doctors and that has to get very expensive
for you. I wish you all the very best. I am 56 btw and was never sick except
for horrendous migraines until the diabetes diagnosis in 1999 then
everything went to heck in a handbasket so to speak. |

Sounds like you need to be clear with them about why you need to see the
MD. Don't mention the UTI until you're in there talking to the endo
about better control. Then you can mention it as an effect of your
being out of control.

him/her... and find someone else. As to having "good" things to tell,
your presence is very good, and you can be a big help to others who
might be going through the same kinds of kidney infections by relating
your symptoms, treatment, etc. here.

sent you back to her. Then, suggest that she get the GP on the phone
right then and there and that the two of them decide which one of them
will treat this.

doctors!!!! They have FINALLY started listening to me for a change,
especially the Infectious Disease Specialist I am now seeing. I have kidney
infections, not bladder infections, so do not have the usual symptoms.
I have also kept up with you throughout the years, so am familiar with your
situation in general. My health beyond my diabetes, plus some real life
situations have prevented me from corresponding as I used to some years ago
in other support groups, but I do think of you regularly, and often wonder
how you are doing. Not glad you had a problem crop up, but glad to hear
from you.
You need a deeper evaluation of your infections. My pcp was only doing a
two week antibiotic treatment, and just before Christmas, we found it took a
month of two stronger ones, and then within ten days infection blasted my
body all over again. They are talking about putting me on iv antibiotics.
This is why you need to get to a urologist (do you have to be referred under
your plans? Not under Medicare do you, I don't.). Because of the way all of
my doctors work together, my endo gets involved with my utis, but only when
I have an undiagnosed one when I am there for a regular visit; he almost
hospitalized me from his office for one last summer, but would have referred
me back to my pcp and urologist for the continuing care.

particularly like it. I also have text messaging, and love to do that!

related at all. That is why she needs to see a urologist, and find the root
of the problem. My bg levels have no effect on my uti infections. Three
consecutive 6.x a1c reports did nothing to squelch the infections. Getting
rid of the infections might make it one step easier to getting the bg levels
down, though.

through the hassle of a referral from the pcp. I don't have to have one for
Medicare, so I don't think she'd have to have one. If she could just see a
urologist, and explain how confused she was from what the doctors had told
her, s/he could give her some dedicated urinary care then, and ease her
anxiety while treating her.

My Endo. sees me only for diabetes and thyroid followup type things. Even
when he suspected delayed stomach emptying, most likely from diabetes, he
referred me back to my GP. |

CRAP!
I guess what you have to do is go to your primary care. When and if
they say they can't treat you because it is caused by your diabetes
then I guess you need to get a recommendation in writing from them to
your endo.
It sounds as if they are trying to pass the buck off to the other one
in the belief that your diabetes being out of control is your fault.
Even if that were true they owe it to you to work with you get back in
control, regardless of what that involves.

I agree. I am going to look for another internist. I went with this doctor
because I knew him when I worked at the hospital and I liked how he treated
his patients there. Also a good friend of mine who is a cop goes to him and
wanted badly for me to see this one so I did and he has me messed up bad. I
had already decided to find someone knew. I hope to find another internist
who will care for the whole body. I am not doing well with endo's for some
reason.

Thank you ! ! ! I wish I had thought of that. I was going to call them today
and ask why I needed my PCP when it was diabetes related but I cried all
afternoon and couldn't talk so I will call on Monday. Thanks... In the
meantime I am going to look for an internist who will treat both hopefully. |

Hi Pricilla,
The endo and her nurse already know all this. It was them that found the
infection to begin with when my PCP told me I did not have one. The endo's
nurse practitioner is the one who said the high BG's are the cause of them
being recurrent all the time.
I feel like no physician wants to deal with me and I really don't know why
because I do try hard.
Thanks for your advice as it is good but I had already done that and right
now the endo has me on Levaquin for the next ten days. |

visit which was about two days later. She said I had it really bad and it
was very concentrated with infection and "junk" as the lab said. So I went
on two weeks of Bactrim. Then went in for speciman and the called and said
it was gone. Then two weeks later they wanted another speciman and said I
had it again so I did another two weeks of Bactrim and went back this past
week which was a week after my antibiotic was finished and now I am on a
once a day antibiotic of Levaquin. I thought sure with the bleeding and the
intense pain I would be in the hospital.

Hello, Diana. You haven't met me but, I have been a lurker for many years
and just recently came out of hiding. I think your endo and your doctor are
saying the same things in different ways. You go to see the endo about your
diabetes and you go to regular doctor for everything else. Just because
diabetes exacerbates certain conditions doesn't mean the endo looks after
it. It is out of his/her expertise. When I had those problems back in the
days before I was in good control my GP looked after my bladder infections
and everything else; my endo looked after my diabetes. Good luck in getting
everything under control. I know it is very depressing when things don't go
right but, remember your prayers will help keep you focused.

I really don't want to get into the complexity of your treatment because
I don't understand the US medical schemes beyond the pay-for sector. But
to me, much of what you have posted points to something being
drastically wrong somewhere.
Do you have any citizen advocacy services near to you? It really does
sound like you need a rip-it-up approach, and for someone with proper
knowledge to take the burden from you and arrange referrals to new
doctors. I think a fresh set of eyes is what's needed.
I do understand that the US public healthcare systems can be very hit or
miss, purely due to geography and complexity, so it may be time for that
approach to happen where you get a referral to a whole new set of
physicians who can basically tear up your notes and do a completely
fresh workup on your health.
As ever you are always in my thoughts.

My thoughts exactly. When I was crying over this mess today I talked to
myself but don't tell anyone LOL but I said those same words to myself. They
are trying to pass the buck. That is why I feel neither of them want to care
for me. We have crummy doctors in my area. Really crummy.
I am glad that God loves me and you and Laurie too. That is what helps me
through the bad times is knowing there are others like you and Laurie out
there and most here in ASD too. God doesn't send others in our life for no
reason. He knows I need alot of love :-) |

Thank you Kate. I don't know why when the Endo's office is the one who said
it was my high BG's causing the infections.
My PCP is the one who told me he could not treat my diabetes any longer. So
shouldn't the Endo be concentrating on doing something to get the bg's down?
I am trying even though some here do not believe me I am doing all I can to
help myself.

Well, if I had something that was a complication of diabetes, I would
expect:
1. I'd probably be sent to the doc best suited to take care of the
complication, most likely not the endo.
and
2. Me and the endo would get more concentrated on good control.
Maybe your endo can recommend a urologist for #1. Or maybe you need to
see your PCP to get such a referral. #1 has more immediate priority
than #2.
I man, my endo pretty much is my PCP but he doesn't do dilated eye
exams on me. He doesn't do my dentistry. He didn't do my vasectomy. But
he did help me or encourage me to find the doctors who do all the other
stuff. He does help me with blood pressure and cholesterol and thyroid
issues, but those are very closely related to my diabetes. He does poke
and prod me in general, but if something came up he'd send me to the
right place. I think that your endo is following the same general
theme. Maybe you can ask for a referral to a urologist?
