Default Should I start Insulin now?

Hi all

I am 41 years old. Weigh 82 kgs. I live in India.
My diet is South Indian Vegetarian food.

My blood sugar levels are
Fasting 95mg/dl
PPBS : 190 mg/dl

HbA1c:5.9

Lipid profile as follows:
Total cholestrol : 209 mg/dl
HDL: 38mg/dl
LDL : 88 mg/dl
Triglycerides : 260mg/dl

I walk 6 kilometers a day.

I want to start insulin, but my GP says since Fasting blood sugar is
within
limits , he advises only physical execrcise and diet restriction only.
Is he
right?

Weight of 82 kg is a normal weight for someone 6 feet tall. Most south
indians are shorter which suggest you are overweight. Insulin will
promote weight gain. Lose weight by eating less. Avoid bread and rice,
eat more beans, dahl, and vegetables and fruits and avoid foods deep
fried in oil.

Weight loss will have a great impact on your glucose levels and aid also
in the lipid numbers. It is the single biggest thing someone at your
stage can do.

You did not mention taking oral diabetic medications, they are the usual
first course to add if exercise and diet modification is failing.

Although any diabetic would benefit from starting insulin at any time,
insulin as the "only therapy" for a Type 2 diabetic is usually not a wise
choice. Your triglycerides are much too high. That means your Insulin
Resistance is too high and that means your risk of heart attack is too high.

You fight high triglycerides with oral meds such as metformin and niacin.
You fight high triglycerides by getting more exercise, daily if possible.
You fight high triglycerides by reducing the level of carbohydrate in your
diet.

Despite his excellent A1c and reasonable fasting BGs his
doctor seems unconcerned at a trigs/HDL ratio of nearly 7
(ideal <3) and post-prandials over 190 (best <140).

If he sees his doctor - I'd recommend he finds a new one.
While he searches, I'd suggest he read this to help with
