| | | enlarged uterus? No big concerns here, just curiosity as to others' experiences.
Just had my pap, and MD says my uterus is enlarged. He felt it was caused
by fibroids, and I have a pelvic ultrasound scheduled to check this out. I
don't really have any symptoms. I am still having periods, at the beginning
of my period, I do have alot of pain, and some intestinal distress. That
has been attributed to IBS. I don't bleed heavily, so we'll see. I more
suspect endometriosis, as I have seemed to had have more symptoms of that
through the years. I am 49, and it might be good to note my mom had
endometrial cancer post menopause. My great grandmother died of a cancer,
mother says endometrial, but it may have been a uterine cancer.
If there are no fibroids found with the sonogram, should I pursue more
testing or just continue with the attitude that this isn't a big concern?
Thanks for your advice...

| | | Re: enlarged uterus? Have you told your Dr about the Cancer History in your family? I would ask
him/her about a Biopsy..just for your piece of mind.
I was diagnosed with fibroids in 1986..and they have never been symptomatic.
My Grandmother died of some kind of female cancer...by the time she went to
the Dr it had spread so bad that they had no idea where it started..this was
in the mid 40's and they probably didn't have allot of info on cancer back
then. I have been in Menopause for 5-7 years...I can't remember when I
stopped bleeding Two years ago during my annual pap the Dr told me she
wanted me to have a Pelvic Ultra Sound because I had to much belly fat for
her to feel my ovaries. The Ultra Sound showed a cyst on my ovary..they
don't like seeing that after menopause. 3 months later I had another Ultra
sound and that cyst was gone but there was one on my other ovary plus they
noticed that my Uterus was extra thick...so they did a Uterine
Biopsy(Ouch...the next time they are knocking me out ) It came back with
Cell changes but not cancer...my Dr said it can take up to ten years for
Uterine cancer to develop..so that is good news. The Fibroids are still
there. The specialist I went to said he wasn't to worried about the cysts
because the Cat Scan showed no changes on the inside of the ovary and it is
still well shaped on the outside..he said only yearly US are needed and if
the ovaries start looking like wild mushrooms they will have to come out. He
didn't seem real concerned about the bleeding..as long as it wasn't heavy
and the Biopsy came back ok. Strange thing though when he left the room his
Resident suggested I have another Uterine Biopsy.

In my case, the problem was extensive endometriosis of the bowel and
reproductive system. I had lots of pain and bleeding and "intestinal
distress". Do a search under bowel endometriosis for more information.
I had a total hysterectomy along with 2 bowel resections and extensive
endometriosis excisions in December 2000 when I was 48. The surgery
gave me my life back.

A few years back I went to see my doctor for some spotting...(turned
out to be nothing)...but he did an ultrasound and while looking at the
screen the nurse said, "What's that?!" And the doc replied "Oh, that's
a fibroid, she has had it for years." He knew it for years, never
told me, I never felt anything, no problems. That was the last time I
went to see him. He never told me about the fibroid and if I hadn't
heard their conversation I would still not know. I had been seeing him
since I was in my 20's but I have a new doctor now.
Anyway, the point of my story is that I never felt anything and
supposedly my fibroid is a large one. BUT...BUT...I don't have the
history of cancer in my family that you have in yours. I would be very
cautious if I were you.

linda,
i am so glad that you had successful surgery & have been able to
reclaim your life. i have friends who have gone through similar
experiences & it's just horrible. & they were not as lucky with the
surgical outcomes. just reading about the bowel resections &
endometriosis excisions made me cringe.
glad you are well,

I remember looking into this when I learned I have fibroids and I read the
incidence of a fibroid becoming cancerous is about 1%. I don't know, though,
if the statistic is higher if there's a history of reproductive organ cancer
in a family. It would be worth asking the doctor.
I shared in another post that I have a very enlarged uterus too due to
fibroids. I had the ultrasound today to measure them (the largest one in
particular). The technician was able to view my right ovary, which is
good...my doctor wanted the ultrasound in part to look at my ovaries...but
the left one was obscurred by the fibroid. I'm waiting now to hear from the
doctor as to her opinion. Tomorrow (later today, really...it's the middle of
the night), I see the new doctor for a second opinion. I'm mostly concerned
with my hormonal problem, but I want to ask him about the fibroid too.

I saw a report on some news show that there is a new procedure to
remove fibroids using a laser ..whatever...if you ever decide to
remove the fibroid ask about that. It's non invasive and supposedly
very safe. But, also very new. If I ever need mine removed I will
ask about it.

I think you might mean ultrasound. A laser wouldn't be noninvasive,
would it?
I definitely heard about the ultrasound one, though. Or maybe it was
supersonic somethingorother. I remember it used sound waves. Focused
ultrasound???
Man, I miss my memory.

You're right...a laser could not be non-invasive. There would have to be a
surgical incision somewhere so the laser could be placed near the thing it's
dissolving. But it would have to be something more than ultrasound too. I
haven't heard of this before, but definitely I'll see what I can find out.
In any case, the second opinion today didn't happen...the doctor cancelled
his afternoon schedule to handle an emergency surgery, his office told me.
The appt. has been rescheduled for Thu. morning. Let's hope this one doesn't
get cancelled. 8 P

The report by Mayo and its collaborators, the Magnetic Resonance Imaging Guided Focused Ultrasound for Uterine Fibroid Group at Brigham and Women's Hospital and Harvard Medical School, appears in the Aug. 1 issue of Obstetrics & Gynecology.

Two different methods. Lasers are not non-invasive. They are an advanced
form of cutting tool, with the advantage of pinpoint accuracy and
sealing blood vessels. Ultrasound does its thing through vibration and
so does not actually cut anything, though it will pulverize tumors and
kidney stones.

I'm on a list for women with fibroids. The interventional radiolists on the
list don't think very highly of this person. He is *not* the only one doing
alternative treatment. Go here for more reputable informaiton: http://www.fibroidsecondopinion.com/

Replying to my own post to follow up.
I got back a while ago from my doctor visit for the second opinion. I have
to say, my first impression of this new doctor is favorable. He was quite
pleasant but didn't make phoney upbeat chit-chat (like my reg. ob/gyn) and
he seemed concerned and sympathetic...and knowledgeable. He talked to me in
his office first and went over my recent history. He agreed I show an
unacceptable intolerance of the progesterine and then he had me go in the
exam room, change into a gown and he gave me an exam. He said that the one
fibroid is huge and he could see justification for removal that would not be
merely cosmetic, although he added if it's not giving me medical trouble, he
could see living w/ it. He told me after menopause, the average fibroid will
shrink by about one third, which is less than I had hoped.
As for the estrogen build-up in utero, he said one approach would be a
hysterectomy, which would effectively resolve both that problem and the
fibroids once and for all. OTOH, he said he sees that as sort of using an
atom bomb to kill a fly, although he added there is sufficient justification
so that it would be covered by insurance. A third option he suggested was to
complete the third course of the natural hormone (Prometrium) since I seem
to do better with that, biopsy the cells again and see what they look like.
If they look better, he said I could try doing nothing further beyond a
repeat biopsy once or twice a year to monitor cell growth. Because I'm
probably fairly close to menopause, he said it's a reasonably good bet the
risk of developing uterine cancer before completing menopause (which greatly
reduces the risk of that kind of cancer) is quite low.
He said he'd need to review my recent tests to give me an intelligent
answer, so he had me sign a consent form to requisition them. He asked me to
call him if I haven't heard back from him in two weeks. Otherwise, if he
gets the records before then, he'll get back to me within a day of that.
The only other thing that happened is the parking garage in the building
(street parking is impossible in that neighborhood) is very hard to
negotiate and I scraped the side of my car against a pillar, which scraped
off a big area of paint. When that happened, an old, forgotten memory was
rekindled: I scraped the side of a past car many years ago--maybe 14--in
that same parking garage against a pillar leaving an appointment in that
same building. And my engine light keeps coming on. Has nothing to do with
anything, it's just pithy.

Really? I had never heard that. Of course, since I am well into
menopause and didn't pay much attention to peri info I missed alot.

Sounds like you found a good doctor. That's great. I am always afraid
that any new doctor I go to will want to correct something....(read:
unnecessary surgery).

Been there, done that....mine was a pillar at a drive thru ATM
machine. It was yellow and left a lonnnnng yellow stripe on the side.
And yes it is pithy.

Which part had you never heard of? The risk of uterine cancer with estrogen
build-up? Yes, that's the risk and why my current doctor wants to treat
it...and therefore put me on the progesterine. Or are you surprised to hear
the risk would be less if I'm close to menopause?
What this doctor said is that uterine cancer is not going to develop
overnight. The estrogen build-up in the lining does put a woman at
risk...but this doctor that once I menopause, my estrogen production will
drop way off, cutting off the build-up and therefore pretty much eliminating
the risk of uterine cancer (at least from that source). What he's saying is,
I could risk doing nothing rather than going on the mini-pill (which will be
a synthetic progesterine again) and have a biopsy ever so often to see what
the cells look like. If I complete menopause in the next year or two,
probably I'll have beat the risk of that particular cancer.

I know! I worry about that too. He does seem like a good doctor, from first
impression.

LOL...it really is. All these little things get to me so much these days.
It's the depression, I'm sure. It's better than it was at first, now that
I've been off the synthetic for almost 2 weeks and since I started the Lexapro. But it's not gone altogether. It's always there, sitting on my
chest like a 20# weight. I wake in the middle of the night or in the
morning, and within seconds, I'm aware of it. And I think to myself, when
will this go away? When will the day come I wake and not feel it?
This afternoon, the little girl I posted about earlier who commented on my
chin hair (two tiny, short hairs), was over to visit for a while. I was
putting in a movie for her and she said, "You have a big waist...and a big
bottom." LOL...that kid really needs to learn some tact.

The part about menopause. I knew the other part. When I was 42 I
stopped having periods...bam...nada...none. Well, I was so excited to
be finished. And it was just like that,WOW. THEN...I went to see my
doctor and told him my good news which he promptly squashed by telling
me that unopposed estrogen was bad...and made me take some progesterone....my 6 months of freedom were over, I started having
periods again. So, I like hearing the cancer risk lowers in
menopause.

I went through the same thing way back when. I finally settled on Prozac 20 years ago. I have been fine ever since. Of course if I stop
taking Prozac I don't know what could happen. But, the pill makes me
feel good, makes life calm and pleasant, help me go through a
separation from my hubby of 40 years with no tears.....I don't plan on
stopping it. Why would I?

I think I would have told her it's not nice to critique Mother Nature!
LOL! My daughter was the same way when she was little. She could see a
flaw at a 100 feet and would promptly announce it to everyone within
earshot.

Yeah, I know. It's not that. I checked that already. Last time it happened,
I took it to the shop and they said they found leaky hoses and replaced
them. I had the car back two days and the engine light came on again when I
started the car and has stayed on since. So I have to take it in today.
And going back to topic...I talked to my ob/gyn's nurse yesterday and she
says my ultrasound looks "stable" which tells me nothing about the size of
the fibroid, which the purpose of the ultrasound was to measure. The nurse
did say the doctor wants to see me back in soon to go over everything. I
haven't made the appointment yet.

Kimmy this is wonderful news. I'd assumed that with your family
history the purpose of having the ultrasound was to see if the fibroid
was rapidly growing. That could possibly mean a cancer. Asymptomatic
fibroids in and of themeselves are not considered to be a problem. Up
to 75% of women have fibroids leading up to menopause based on
ultrasound screenings studies. Most women are not even aware that they
have them.

I think you may have me confused with another poster. I don't have any
family history that predisposes me to uterine or ovarian cancer. The purpose
of the ultrasound was to measure the fibroid because it's so large, and to
see if my ovaries could be viewed, just for annual evaluation. The right one
was viewable, but not the left (which happened last time). The left was
obscurred by the fibroid.
I know that fibroids are not considered dangerous. Mine is not really so
much a problem, other than size. It's not painful, but I do have a very
enlarged belly due to it. I look like I'm 6 or 7 mos. pregnant. That's why
my doctor says removal would be only cosmetic, therefore not recommended
when I'm so close in age to menopause. OTOH, she said she wanted to get an
exact measurement to say for sure. Maybe when she said it's stable she means
it hasn't grown much since last measured. Maybe I'll know when I see her,
but I don't know when that will be.
