Default frightened about Osteoporosis

About 5 1/2 years ago my spinal T score was -2.4 My doctor put me on
Fosamax and two years later it was -2.1

Since I've been on Fosomax for more than 5 years the doctor considered
taking me off for a while. However she wanted a scan first. The T
score for my spine is now -2.6 which means I've gone from Osteopenia
to Osteoporosis despite the Fosomax. She decided to keep me on the
Fosomax and rescan in 2 years.

I have a serious back problem and also a lot of pain in my knees.
Therefor I cannot do the bone strengthening exercises that might help
me. The only exercise I can do is swim, which I do for 40 minutes
every day but I know that doesn't help my bones.

I am 65. I have a friend in her early 80s who is so disabled by
Osteoporosis that she is in a nursing home where she is barely able to
get around in a wheel chair. She is in pain all the time. Her spine
is crumbling. This scares the hell out of me. Her O. wasn't
diagnosed until it was very far along, but still, if my T scores are
getting worse despite the Fosomax what's in my future?

I'm really frightened.

IMO you ought to find a physical therapist who can teach you weight-bearing
exercises, or ways to perform weight-bearing exercises, that will not hurt
you. For example you might be able to use certain gym machines but not
others. Or you might be able to walk if you wore knee supports, knee
straps, etc. You already recognize that lack of weight-bearing exercise is
contributing to your problem. There is something you can do about it. Make
the call, you have very little to lose and a lot to gain. Good luck.

I'm glad you replied, because I wanted to say something helpful, but had no
clue! (Although I think I do now remember you & someone else mentioning PT
for osteoporosis in the past at some point, saying that it can help quite a
lot.)

I already did this. Several years ago. The exercises that were tried
all ended up causing my back to go into spasms. As for knee supports,
the problem is with the nerves. I just started Neurtontin. If that
ends up helping then I'll be able to do some walking, but right now I
can't do more than walking around the super market.

I can't offer much but sympathy - it's a concern for many
women but your circumstances make it harder.

I can also point out a new article that came out in the
_New York Times_ just a couple days ago, and might
offer some degree of help, from a dietary POV:

Well, while you're at the supermarket, load up on fruits and vegetables,
because apparently there is now some evidence that eating a lot of them
leads to better bone health too (I just came across this article this
morning -- http://www.nytimes.com/2009/11/24/he...d.html?_r=1&em ).

Other than that, I don't have any more suggestions, except to wish you good
luck.

I get back spasms from dental visits, needle sticks or any time I'm low
on potassium due to adrenal insufficiency. Some time released potassium
chloride prevents them and leg spasms completely for me, it could help
you. When I'm severely adrenally insufficient, any stress, even things
that don't upset me, like a dental exam, a needle stick, will cause back
spasms, and potassium prevents them or lessens them.

Eva is right, finding a way to do weight bearing is key.

Is it possible that you have badly controlled diabetes and/or insulin
resistance? That can cause a lot of what you describe. Even if
undiagnosed, you could be a very advanced diabetic, since the fasting
glucose screen or HbA1c will fail to diagnose diabetes until it's very
advanced.

Stop the fear.
A lot can happen in 15 years. Osteoporosis doesn't kill. You might drop dead
of something else long before then. If you live to be 80+ and your only
worry is a "crumbling" spine, be glad.

I always find myself wondering how beans fit in with this. They are both
a vegetable and a protein.

I thought legumes weren't veggies?

With bone mass, it appears that no one food group is determinative. I
recall reading studies in which elderly folks added and maintained the
best BMD with more animal protein. I think a combination of quality
proteins and lots of veggies with adequate fats for absorption of
nutrients from veggies may be ideal:

http://www.ajcn.org/cgi/content/abstract/81/6/1423

The references under the abstract are interesting, too.

An awful lot of authors confuse brief increases in urinary calcium with
bone loss, but it's been proven untrue over and again.

Botanically, if it's a member of the plant kingdom, it's "vegetable."

Legumes are the seeds that result from fertilizing a flower of a
particular type of plant. http://en.wikipedia.org/wiki/Legume

Culinary definitions are a different matter :-).

Either I'm humor-impaired tonight (is it a joke?) or ... Nuts are
seeds, just like legumes, except they grow on trees. Or do you
mean the ... human ... type of nut3? (Take pity on the woman who
had really bad insomnia last night!)

Well, for me, there is a simple difference: Peas and beans don't give me a
stomach ache. Nuts do.

This is because once I start eating them I can't stop. Not true of peas and
beans.

First: I hope you get some sleep tonight! A few months ago I actually slept
well (for me; went to sleep after reading for about 45 min. - 1 hr.) for
about 2 months straight. Yea!!! I was thrilled! In the past it's relented
for a week - or two, at the most - a few times, & then came back. So...
thought the post-meno insomnia crap had *finally* vamoosed. But then...
would zap awake in the morning feeling slightly panic-y over absolutely
nothing & the insomnia kicked back in - it's so very obviously a physical
thing. Grrrrr...

Second: Not really a joke -on my part, at least, just a matter of "keep it
simple" works for me in distingusihing a fruit from a vegetable in everyday
life. It was getting mighty complicated as to what belonged in which
category!

Third: liked your reply, though - the human type of nut hadn't even entered
my brain. ;-)

Yep....hey, do you remember when we were kids they used to dye the pistachio
shells red for some reason? And that dye used to get all over your fingers

Hey, you're right - I'd forgotten all about that! Wonder what the purpose
was behind the dye job? (And heaven only knows what sort of red dye it
was!!)

Heaven knows is right. Those pistachios of my childhood were not grown in
California, but imported from Iran!

I didn't know that.

I've been getting increasingly concerned about all the food items
that are imported from China. (Although a lot of the food that
originates in the US is also produced under very dubious
conditions...) Then today, I opened up a box of tea from India,
and the phrase, "... not for all the tea in China..." came to
mind. I've been drinking Chinese tea for a long time, I guess!

Loved 'em.

I'm pretty sure I was about 30 y.o. before the last of that dye wore off.

Betcha it was the "green = unripe while red = ripe" concept.

Yeah, maybe that was it. Or just that the color red catches the eye -
whereas their natural color isn't nearly as eye-catching - you don't notice
them so much, & would be more liable to pass them by. Had never thought
about it before. ;-)

I was both gladdened and saddened the first time I bought shelled
pistachios. Gladdened, because I no longer had to break my nails
and stain my fingers trying to open them. Saddened, because do
you have any idea how quickly a pound of shelled pistachios
disappears???? (Speaking of stomach aches...)

That's why I don't dare buy them shelled.

Only way to eat nuts is to take a serving out of the bag and put the bag
away. At least you have to *think* about pigging out if you take it out
again. :-)

It is easy to make a gluten-free and/or Passover flour from the shelled
ones (at about 1/5 the price of buying pistachio flour).

I've never heard of that! I have almond, hazelnut flours in my freezer,
but have never seen pistachio flour.

They're common in German baking, I think, and low carbers use them to
make stuff, especially as a sub for graham cracker crusts and cookies.

Almond flour is the most common, and you can get it toasted or raw.
Coconut flour is another high fiber, low carb flour sub.

But pistachio flour is news to me, too!

Thanks for the explanation. Thinking of the crumb-type crusts - esp. the
ones that contain super-finely chopped nuts, makes sense now.
