Actually, I should clarify: what Cicherillo means by a "flat wrist"
seems to be pronation, not supination! (Pronation is when your fist,
when pumping your biceps, is facing away from you -- right?)
Unfortunate word choice on his part, but that's what he appears to
mean by the term.

So, he recommends doing dumbbell curls using a pronated grip! And it
does seem to make a difference!

I seem to doing the pinky-higher thing naturally, which is why I've
not noticed things before, I guess...BTW, it's hard to look straight
into the mirror while doing them bent-over lateral raises for the rear
delts!

But playing with the positions -- pinky-higher-than-thumb or not --
without any weights in my hand, I seem to only sense muscle movement
(a difference) in the arms, not the rear delts!

Also recommended for the bent-over rear delt lateral raise is to keep
the wrists lower than the elbows (I think this is Arnold's point).
Again, I tend to do this naturally, particularly as the exercise
progresses and I lose strength, but I don't seem to notice any
difference in terms of muscles worked or such.
